That handler is called when you quit the game, for every single solitary bleeping atomic particle in your inventory.
Prove it to yourself by putting a debug message in the on_item_drop handler. Check your available disk space first.
Check out the ZRP demo; I recall handling that for medkit drops.
I don't know about loads; I wouldn't think that handler would be called then -- except perhaps indirectly, because you are doing something that swaps a weapon out...
